It depends on the audit. Some pages need a genuine rebuild; others just need the offer, form or trust signals fixed. I recommend the smallest change that actually solves the problem.

Can you test changes before committing to them?

On sites with enough traffic, yes. On lower-traffic sites, formal A/B testing rarely reaches significance in a reasonable time, so I rely more on established conversion principles and direct user feedback.

Does this include the ad campaign, or just the page it lands on?

Just the page and what happens after the click, though I flag it clearly if the actual problem is upstream in targeting or ad creative.
